Preventing falls in a 350-bed community hospital

Increase in defect-free audit compliance
+76%
Falls reduction after just 6 months
-90%
Each fall in a hospital results in $33,000 in average costs, which is just one reason healthcare organizations seek easy tools to help staff prevent patient falls.

In one of our community hospital customers, electronic medical records indicated good falls prevention practices were happening, but leadership had no way of showing what actual practices were actually being used by staff. Without knowing what practices staff were, or were not, following, management had no way to focus training and even further improve best practices to keep patients safe.

Staff collected data on compliance with 14 different best practices to prevent falls, from ensuring call buttons are within reach to using slip-proof footwear. By using that data to target improvement efforts where they were most needed, the hospital achieved greater than 90% compliance with all 14 practices, and defect-free audits increased by 76%.

After just 6 months of using Qualaris, the hospital saw a 90% reduction in falls, and they sustained a 56% reduction over the entire fiscal year, not only avoiding huge costs but helping to keep their patients even safer.

Preventing Infections In A 100-Bed Community Hospital

Improvement in CAUTI bundle compliance

Improvement in Hand Hygiene compliance
+35%
Nearly 1 in every 20 patients acquires an infection during their hospital stay, at a cost of nearly $25,000 per infection. This small community hospital was looking for infection prevention tools that were efficient and enabled them to do more with their limited resources.

Best practices for improving hand hygiene are not the same as the practices for CAUTI bundle compliance, but Qualaris provided a transferable platform that could be used by management to audit the best practices in both. Eliminating paper processes and shifting auditing to a single software platform saved their nurse managers 10 hours per month, giving them more time to focus on their other diverse responsibilities.

Even more, their nursing teams saw dramatic improvements in compliance: 72% improvement in CAUTI bundle compliance and 35% improvement in hand hygiene compliance. That means fewer infections, huge cost savings, and most importantly, safer patients.

Improving Patient Communication in a 250-bed hospital

Reduction in incomplete whiteboard elements
-84%
Qualaris isn’t limited to just preventing adverse safety events. Research shows that when patients and their families have clear information, they are more engaged in their care, more compliant on discharge instructions, and achieve better outcomes.

A Western Pennsylvania hospital recently developed and installed new, patient-facing whiteboards to improve that communication and patient experience. In rolling out the new whiteboards, the leadership team wanted to ensure that staff were consistently complying with the process.

Qualaris provided the tools to ensure whiteboard compliance and improve rounding and patient communication. With nurse managers using tablet computers to audit for compliance and focus training opportunities for staff, the hospital has achieved at least 90% compliance with all 17 whiteboard elements along with an 84% reduction in incomplete whiteboard elements. Now, they are exploring ways to use Qualaris to improve other quality processes.

Streamlining Process Improvement for a 300-bed hospital

Improvement in patient communication best practices
+43%
Qualaris’ Audit tool provides flexible designs, which made it a great fit for a 300-bed hospital that wanted to track and improve processes across patient experience, quality, and patient safety work.

The most pressing priority was to create an inpatient leader rounding tool to understand and improve patient and family experiences. By identifying missed opportunities for staff to communicate clearly with patients and their families, clinical managers were able to focus on the biggest improvement opportunities, leading to a 43% improvement in staff compliance with patient communication best practices.

After seeing how easy it was to launch new solutions, the hospital began to hold brainstorming sessions to identify emerging needs to address through auditing. With no IT integration needed, the team could design a new audit in as little as 15 minutes and have it launched and in use rapidly. Now, this hospital alone has more than a dozen different Qualaris applications live and in use, including rounding tools for perioperative services, infection control audits, and patient and family survey tracking.

Collaborative Hand Hygiene Improvement For 12 Hospitals

Collaborative-wide hand hygiene compliance after 1 year

Qualaris is designed to also support multi-site programs and collaboratives. By making it easy to share evidence-based templates between many sites, automating benchmarking reports, and providing experienced support for implementation, large groups can learn and improve faster together.

Working with the Healthcare Council of Western Pennsylvania, Qualaris helped this regional association work on improving hand hygiene together. To start, the group developed a template for hand hygiene auditing with the help of their hospital members. Qualaris assisted the association in helping participating hospitals locally customize and implement the quality improvement tools through interactive webinars. Soon, the whole region was equipped to measure, analyze, and improve hand hygiene compliance and share and compare their results through collaborative meetings.

Over 12 months, this 12 site collaborative successfully moved their hand hygiene programs off of paper and spreadsheets, saving staff time and providing timely data. Using these benefits, the collaborative’s average hand hygiene compliance improved from 78% to 94%.

Reducing CLABSI Rates For A 500-Bed Academic Medical Center

Qualaris helped a 500+ bed academic medical center improve compliance with central line maintenance bundles from 53% to 86% while reducing their cancer center’s central line associated bloodstream infection rate to 0 for multiple months.

At this major medical center, the organization had enjoyed previous success in reducing their CLABSI rates. However, infections spiked back up a year later. It was determined that high performing units were carrying out regular audits to ensure that central line maintenance bundles were reliably being completed. Using Qualaris' software, the organization was able to quickly design, test, and iterate Audit tools that were used by unit managers across the organization to measure and improve compliance.
